Your expertise helps advance our mission of making communities healthier®. More about our teamOur Skilled Nursing & Rehabilitation Unit includes 90 beds-20 dedicated to short-term rehabilitation and 70 to long-term care. Our interdisciplinary team includes Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), Medication Aides, and therapy partners who work collaboratively to provide high-quality, person-centered care.As a Wound Care RN, you will serve as the clinical expert for residents with wounds, collaborating closely with the Director of Nursing, physicians, and the interdisciplinary team. You will assess, plan, implement, and evaluate wound care treatment plans while supporting education, quality improvement, and best practices across the unit. How you'll contributeA Wound Care RN who excels in this role:Performs initial and ongoing wound assessments, including staging, measurements, and documentationIdentifies changes in healing progress and communicates findings to physicians and nursing leadershipImplements prescribed wound treatments and evaluates resident responseParticipates in weekly Quality Improvement sessions and interdisciplinary care planningCollaborates with nursing staff to identify resident needs and develop individualized care plansProvides education to residents, families, and direct care staff regarding wound care and preventionCoordinates with outpatient wound care facilities and ensures continuity of careMaintains accurate, timely, and confidential documentationEnsures proper infection control practices and equipment maintenanceSupports harmonious team relationships and promotes a positive care environmentMaintains clinical competency through ongoing education and training Why join usWe believe that investing in our employees is the first step to providing excellent patient care.
