Minimum Requirements Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.Active Texas Physical Therapy license required to start the assignment.Current BLS certification from AHA or ARC may be required.Home health experience required. Candidate must have experience with home health evaluations, start-of-care visits, 30-day reassessments, treatment visits, and discipline discharges.Experience with HCHB (HomeCare HomeBase) electronic medical record is required or strongly preferred.Ability to travel within coverage areas including Crockett, Grapeland, Elkhart, and Palestine during the assignment.Minimum education requirement: Bachelor's Degree as noted by the client.Professional Physical Therapy license required. Assignment Details Facility Type: Home Health.Shift Name: Days.Shift Details: Days 0800-1600 (8:00 AM - 4:00 PM) with schedule indicating 5x8.Weekly Hours Guarantee: 36 hours per week.On-call/Weekend Requirement: none.Coverage Area: Candidate should expect patient visits in Crockett, Grapeland, Elkhart, and Palestine; mileage between visits is reimbursed.
