Behavioral Health Care Manager (LMSW/LCSW) – SNF | CoCM ProgramOverview:We are an established SNF physician group seeking a Behavioral Health Care Manager (BHCM) to support our Collaborative Care Model (CoCM) program across Skilled Nursing Facilities (SNFs) in Georgia. This role focuses on managing residents with depression, anxiety, and other behavioral health conditions through a structured, team-based model in partnership with primary care providers and a consulting psychiatrist.This is a high-impact, population health role centered on resident engagement, care coordination, and outcomes tracking.Responsibilities:Conduct resident outreach and follow-up (primarily telephonic/virtual; coordinate with on-site SNF staff)Administer and track PHQ-9, GAD-7, and other screening toolsMaintain a behavioral health registry and monitor clinical outcomesProvide brief, evidence-based interventions (behavioral activation, supportive counseling, motivational interviewing)Collaborate closely with SNF nursing staff, PCPs, and care teamsPresent cases during weekly psychiatric case reviews and implement recommendationsTrack and document time and services in compliance with CoCM billing requirementsEnsure accurate, timely, and compliant EMR documentationQualifications:Active Georgia license as:LMSW (under appropriate supervision) orLCSW (independent practice eligible)Experience in behavioral health, SNF/long-term care, or care management preferredStrong communication and interdisciplinary collaboration skillsAbility to manage a high-volume caseloadComfortable working across multiple facilities and care teamsProficiency with EMRs and documentation workflowsWhat Success Looks LikeHigh resident engagement and follow-up complianceImprovement in PHQ-9/GAD-7 scores and behavioral health outcomesStrong collaboration with facility staff and providersAccurate documentation supporting CoCM billing and quality standardsWork Environment:Hybrid role supporting multiple SNFs across GeorgiaTeam-based model including:Primary Care Providers (MD/DO/NP/PA)Consulting PsychiatristSNF nursing and administrative staffCompensation & GrowthCompetitive salary based on licensure (LMSW vs LCSW) and experienceGreat clinical resources, training and structured supervision availableOpportunity to grow within a scalable, team-based behavioral health programWhy Join Us:Help bring behavioral health access to underserved SNF populationsWork within an innovative, evidence-based care modelBe part of a rapidly growing program with strong clinical support
