Job DescriptionThis is a work from home position that can be performed from anywhere in the United States.Work hours 8:00-4:30 in time zone of residence. Coverage for late shift as needed (8a-4p EST)As a Registered Nurse (RN) UMNC on the Care Transition team, you will optimize our members' level of wellness and independence by avoiding unnecessary hospitalizations or service duplication, provide continuity of care, cost containment, and improve clinical and financial outcomes resulting in improved quality of life. You provide appropriate education and clinical inventions to members in multiple disease populations. Develop deep relationships with members and/or their caregivers functioning as a resource and advocate to enhance quality of care. You are the link between the member, the providers, the payer, and the community. The UMNC in this role will use their excellent written and verbal customer service skills and advanced computer skills in order to interact with key partners and members. As a Care Transition Nurse, you will use your clinical expertise to address social determinants of health issues, facilitate care gap closure by providing education, connection to resources, and referrals to the plan care management program for ongoing support as needed. The Care Transition UMNC utilizes advanced clinical judgment and critical thinking skills to facilitate appropriate member physical health and behavioral healthcare through direct provider coordination/collaboration, and coordination of psychosocial wraparound services to promote effective utilization of available resources and optimal, cost-effective outcomes. Work autonomously and collaborate with multidisciplinary/multi-setting healthcare teams to coordinate quality, cost-effective care and minimize fragmentation of the healthcare delivery system.Typical office working environment with productivity and quality expectationsWork requires the ability to perform close inspection of hand written and computer generated documents as well as a PC monitorSedentary work involving periods of sitting, talking, listening. The actual salary offer will take into account a wide range of factors, including location.Required QualificationsActive and unrestricted RN license in state of residence5+ years of experience as an RN Ability to work hours of 8:00-4:30 in time zone of residence.
