Work history must include name of facility worked, city and state, title, department, and dates employed from the last 7 years. Agency names are not accepted as the employer. Highest level of education listed, including month/year of graduation and degree achieved if applicable.Skills ChecklistCompleted within one (1) year of submittal for specialty where candidate is submittedTwo (2) Approved ReferencesTwo (2) professional references from work history occurring within the past three (3) years. One must be from a supervisor (charge nurses are considered supervisors). Reference must include the full name (first & last name) of the person providing reference. The name of the facility and the dates of employment must match the resume. Reference must include a breakdown of skills and strengths with a rating score.
