We re hiring experienced Nurse Practitioners to join a growing team that specializes in in-home risk adjustment and wellness assessments for Medicare and other vulnerable populations.This is an opportunity to make a measurable impact in value-based care, helping ensure accurate documentation, care gap closure, and individualized health education all from the comfort of your patients homes.What You ll DoConduct in-home annual wellness visits and risk adjustment assessmentsEvaluate patients with chronic conditions through:Comprehensive physical examsMedication reviewsCognitive, mental health, and fall risk screeningsSocial and functional assessmentsEducate patients on health maintenance topics such as medication adherence, pain management, and chronic disease self-careHelp close quality care gaps (labs, screenings, immunizations)Accurately document assessments in a user-friendly EMRQualificationsActive, unrestricted Nurse Practitioner license in OR3+ years of experience in patient care (geriatrics, primary care, or home health preferred)Previous experience in risk adjustment or in-home assessments preferredComfortable working independently in a field-based environmentMust have reliable transportation and a willingness to travel
