Job Title: Dental Outreach Coordinator (DOC) Date Prepared: April 2026 Location: BACHC EEO Category: Clerical Department: Dental Non-Exempt Reports to: Dental Director JOB SUMMARY: The Dental Outreach Coordinator (DOC) for the Community Based Dental Services Program provides coordination of care for all off-site dental events and serves as a case manager at select schools and programs within the Community Health Systems (CHS) Seal a Smile Program and other outreach program collaborations. This role has the primary goal of ensuring the entire program runs smoothly at community-based locations and specifically targeted schools. This includes both direct outreach, parent / caregiver engagement, and timely follow up to address site specific needs. This role will work directly with the Dental Director and the Dental Practice Manager to coordinate providers and dental assistant resources as needed. The DOC is principally responsible for ensuring that needed paperwork, consents, and schedules are prepared prior to the occurrence of an off-site dental event. The Dental Outreach Coordinator is responsible for all reporting and compliance related requirements based on contractual, state, and federal expectations. This includes ensuring compliance with HRSA Standards as well as Seal-A-Smile protocols and reporting requirements and CHS policies and procedures. ESSENTIAL JOB FUNCTIONS: * Work directly with the Dental Director to develop an operational plan for the provision of year-round, community-based dental and hygiene services. * Become an inclusive contact for the Wisconsin Seal-A-Smile program. * Ensure that annual reporting requirements are completed * Ensure that appropriate invoicing is completed. * Ensure that the annual request for proposals is completed. * Serve as primary contact with the schools for initial outreach and relationship development. * Provide on-site assistance for student/patient management while Seal-A-Smile services are being performed. * Work with the dental hygienist to ensure that workflow needs are being met. * Serve as the primary project coordinator for all community based dental services. * Community based dental services to include for the following partners: WIC, Head Start and nursing homes. * Coordination of the provider's schedules for these services and assuring the full circle of schedule coordination is done in Epic and Teams and email communications to community partners. * In conjunction with the DPM, develop an annualized plan for the performance of the Seal-A-Smile services. * Ensure that all records and data entry are managed in accordance with CHS standards. * Work with the Outreach and Enrollment Team to Assist eligible families to enroll in Medicaid which not only includes dental coverage but medical coverage for the child and possibly the entire family. * Educate children and parents about the importance of oral hygiene and regular dental visits. * Assist clinical teams in the field with computer needs, gathering students/patients, moving equipment and other equipment needs. * Regular check-ins and reporting to Dental Director * Other duties as assigned. PREFERRED QUALIFICATIONS: Skills/Abilities: * Strong organizational skills and ability to independently manage various projects and duties. * Ability to track and report compliance related requirements. * Comfortable speaking to organization personnel, children & parents. * Strong communication (written and oral) skills * Ability to manage project implementation. * Ability to set and manage programs thru the use of established Key Performance Indicators. Education: * Associate's degree in healthcare or a related field preferred. * Dental experience preferred. Related Work Experience: * Administrative experience preferably in public health or community health a plus. * Prior experience working with patients/families in need of community-based healthcare.
