We have been an established practice since 1997, and we take pride in providing our community with high-quality, patient-centered healthcare.Our Values: Put Patients FirstEmpower Entrepreneurial Provider and Care TeamsOperate with Integrity & ExcellenceBe InnovativeWork As One TeamWhat You'll DoEvaluate, diagnose, and treat a broad range of adult medical conditions, including chronic illnesses such as diabetes, hypertension, COPD, and heart disease.Conduct routine and preventive care visits, screenings, and wellness checks to promote long-term health and early detection of disease.Order, interpret, and follow up on diagnostic tests and lab work.Develop evidence-based, personalized care plans in collaboration with patients and their families.Manage a continuity panel of adult patients, ensuring appropriate follow-up and long-term care.Document patient encounters, diagnoses, and treatment plans accurately and promptly in the electronic health record (EHR) system.Coordinate care with specialists, behavioral health professionals, and other providers as needed.Participate in quality improvement efforts and initiatives tied to clinical performance metrics and value-based care goals.Deliver culturally sensitive and linguistically appropriate care tailored to Houston's diverse community.QualificationsCompletion of an accredited Internal Medicine residency programBoard Certified or Board Eligible in Internal MedicineUnrestricted and active Texas Medical License (M.D. or D.O.)Current DEA registration (Texas)Current BLS and ACLS certificationsProficiency in Spanish or another second language is a strong plusExperience in a value-based care or community clinic setting is preferredExcellent clinical judgment, communication, and teamwork skillsPassion for delivering high-quality, patient-centered care to underserved populationsBilingual Spanish preferredEnvironmental Job Requirements and Working ConditionsThis position is located in-office. This salary range represents our national target range for this role.
