They are trained and experienced in long-term care facility regulation, standards of practice in long-term care, and evaluation of patient care. They conduct on-site monitoring visits to facilities with a history of patient care deficiencies or for those that are identified as high and medium risk through the early warning system. The dietitian quality monitor uses independent, professional judgment and clinical expertise to provide technical assistance to long-term care facility staff, independently and as a part of a Rapid Response Team to assess the overall quality of life and quality of care of individuals residing in long-term care facilities. The dietitian quality monitor utilizes expertise in the area of nutrition to assess care systems through observations, interviews and clinical records review to identify facility systems strengths and opportunities for improvements in patient care and services rendered to residents. The observations and comprehensive analysis of all findings may result in recommendations to administration staff for procedural and policy changes. The dietitian quality monitor provides in-services education and staff trainings based on evidence based- best practices that help facility staff implement processes that results in improved health outcomes for individuals. Essential Job Functions: * Attends work on a regular and predictable schedule in accordance with agency leave policy and performs other duties as assigned. Works collaboratively across MCS to identify innovative and effective solutions. * Plans, prioritizes, and performs a variety of technical, training, research, planning, policy, program assessment, and administrative activities during on-site or off-site quality monitoring and rapid response team visits including assessment of the delivery of services; provides high-level technical assistance to providers' staff; and creates efficiencies where opportunities occur. (40%) * Coordinates the collection, organization, analysis, and preparation of materials by providing complex technical assistance, in-services education and other trainings in response to nursing provider requests for nutrition related information and reports. (20%) * Serves as the lead on rapid response teams to coordinate and monitors facility systems through observations, interviews and record reviews identifying facility strengths and implementation of changes and/or opportunities for improvement.
