As an RN, you will oversee the coordination of patient care from admission to discharge, ensuring that all patients receive efficient, effective, and high-quality care. You will collaborate with physicians, nurses, and social workers to optimize patient outcomes, reduce length of stay, and manage hospital resources effectively. Job specificationCandidate requirementsJob specification Lead and manage the case management department, including utilization review, discharge planning, and care coordination. Develop and implement strategies to improve patient flow and reduce hospital readmissions. Ensure compliance with regulatory requirements, accreditation standards, and hospital policies. Monitor departmental performance, set quality and financial goals, and report on key performance indicators. Recruit, mentor, and develop case management staff to support the delivery of high-quality care. Engage with patients and families to address concerns and provide education about care transitions. Candidate requirements 5+ years of nursing experience, with at least 2 years in a case management or leadership role. Knowledge of case management, utilization review, and discharge planning processes. Strong leadership, communication, and organizational skills. Bachelor's degree in nursing (BSN) required; MSN or related advanced degree preferred. Case Management Certification (CCM) or ACM certification is preferred.
