Job Description

Job Summary: The Mental Health Counselor will be responsible for providing case management and counseling services to address the bio-psychosocial needs of patients at the Community Health Center of Buffalo, Inc. Responsibilities: * Conduct assessments to identify patients' psychosocial needs and develop appropriate treatment or referral plans, including case management, short-term counseling, and crisis intervention. * Assist patients in navigating entitlement programs, supplement insurance, and other benefits to reduce reliance on emergency assistance. * Aid patients in accessing prescription cost assistance programs and community resources. * Act as a patient advocate when interacting with community agencies. * Maintain accurate patient records, prepare reports, and ensure proper documentation practices. * Arrange transportation assistance for patients when necessary. * Participate in clinical meetings and case conferences to ensure holistic care for patients. * Address patient education and support needs, including pregnancy counseling, parenting skills, and caregiver support. * Conduct assessments and provide education to prenatal and post-partum patients. * Collaborate with healthcare providers and make referrals as needed. * Evaluate and cater to the unique needs of special population patients such as the homeless, substance abusers, and at-risk adolescents. * Offer HIV counseling, testing, and follow-up care for patients. * Monitor and follow up on missed appointments with patients. * Contribute to Quality Improvement and Quality Assurance initiatives. * Maintain a clean, safe, and organized work environment. Qualifications: * Licensed Mental Health Counselor (LMHC), Licensed Social Worker (LSW), or Licensed Certified Social Worker (LCSW). NYS Permit holders are encouraged to apply. * Three (3) years of experience as a Mental Health Counselor or Social Worker preferred, with strong interpersonal and communication skills. * Experience with trauma-informed care and EMDR Therapy is advantageous. * Proficiency in MS Word, Excel, and Outlook. Physical Demands: This position involves periods of sitting, standing, walking, bending, and stooping. Weekend and evening availability may be required. Salary Range: $52,000 - $58,000
