Job Description Responsibilities: • Provide comprehensive medical care to patients aged 6 months and older in ambulatory care settings. • Conduct consultations by examining patients, reviewing medical records and reports, researching information, and developing care recommendations. • Adhere to professional standards, practice policies, federal, state, and local requirements, and regulatory guidelines. • Collaborate with other providers to develop diagnoses and treatment options. • Prescribe or administer treatment, therapy, medication, vaccinations, and other specialized care to prevent or treat illness, disease, or injury. • Order, perform, and interpret tests; analyze records and examination information to diagnose patient conditions. • Record and maintain patient information, including medical history, reports, and examination results. • Monitor patients' conditions and progress, adjusting treatments as necessary. • Review health risk assessments (HRAs) to help determine care plans. • Explain procedures, test results, and prescribed treatments to patients. • Refer patients to specialists or other practitioners when appropriate. • Participate in implementing a multi-disciplinary, patient-centered medical home approach. • Stay current by engaging in continuing medical education, reading professional publications, maintaining networks, and participating in professional organizations. • Perform other duties as assigned. Qualifications: • Graduate of an accredited medical school and licensed to practice in New York State. • Board certified or board eligible. • Minimum of 1-2 years of experience preferred. • Ability to travel to other sites for coverage as needed. • Bilingual in English and Spanish or Creole/French preferred.
