Community Clinician – ECCS (Hybrid)Chicago, ILFull-Time | Hybrid | $3,000 Signing BonusThis is a field-based, hybrid position. Through individualized, community-based services, we strengthen family systems and promote long-term stability.We are expanding our Enhanced Clinical Community Support (ECCS) Program and are hiring Community Clinicians to join this highly collaborative, mission-driven team.For a limited time, we are offering a $3,000 signing bonus.About the ECCS ProgramThe Enhanced Clinical Community Support (ECCS) Program provides immediate, short-term, intensive clinical services to stabilize youth in DCFS care following inpatient psychiatric hospitalization. Services are delivered in the home and community and focus on strengthening caregiver capacity, improving system coordination, and reducing the risk of placement disruption.About the RoleThe Community Clinician is a unique, advanced clinical role that blends direct clinical intervention, caregiver coaching, consultation, and advocacy. Clinicians partner closely with caregivers, caseworkers, and multidisciplinary teams to ensure continuity of care and developmentally appropriate, trauma-informed support for youth.What You'll DoProvide direct clinical support and consultation to caregivers, caseworkers, and multidisciplinary teams for a caseload of 7–10 youthConduct weekly in-person caregiver support sessions focused on trauma-informed psychoeducation and therapeutic parenting strategiesOrganize and facilitate monthly clinical team meetings with youth, caregivers, caseworkers, and other involved providersMaintain consistent communication with assigned casework agencies and collateral partners to identify youth needs, family needs, and system barriersCollaborate with child and family teams to develop, review, and update individualized service plansUtilize trauma-informed and evidence-based clinical models, including ARC, SPACE, SMART, Theraplay, or other appropriate interventionsProvide ongoing clinical consultation to caregivers and professionals supporting the youthSupport families with clinical case management, including referrals for therapy, psychiatry, and other servicesAdvocate for youth and family needs in DCFS clinical staffings, CFTMs, school meetings, and other systems spacesEnsure timely and accurate documentation, including progress notes, treatment plans, CANS 2.0, and discharge summariesConsult regularly with supervisors and team members to ensure youth safety, permanency, and well-beingTravel throughout Cook County for in-person sessions with familiesScheduleFlexible full-time scheduleEvening availability required (e.g., 10:00am–6:00pm or 11:00am–7:00pm)Hybrid role with a combination of community-based and remote workWhat You NeedMaster's degree in Social Work or a related social service fieldLCSW or LCPC preferred (not required)Experience working with youth involved in child welfare and/or complex traumaStrong clinical judgment, organization, and communication skillsComfort using Microsoft Word, Outlook, and video conferencing platformsValid driver's license, insured vehicle, and ability to travel throughout Cook CountyWhat It's Like to Work HereKaleidoscope is a supportive, mission-driven organization.
