SummaryJob Description:Job DescriptionPosition SummaryThe pharmacy residency program is a twelve-month postgraduate training program designed to develop a skilled practitioner who is prepared to deliver patient-centered pharmaceutical services and interact with members of the health care team accountable for the care of patients with complex conditions in a community hospital environment. The resident will also be prepared to pursue postgraduate year two (PGY2) training if so desired. The resident will be responsible for supervising and performing in all pharmacy operations in accordance with accredited hospital policies and procedures, ethical and professional practices, The Joint Commission standards, and State and Federal requirements. The resident will act in compliance with the ASHP accreditation standard for postgraduate residency programs. Pharmacy Residents are responsible and accountable for the provision of safe, effective, and prompt medication therapy. Through various assignments within the department, they provide support of centralized and decentralized medication use systems as well as deliver optimal medication therapy to patients with a broad range of disease states.All pharmacists are responsible for drug preparation and distribution for medications; evaluating, inputting and verification of medication orders; assisting providers and other health care professionals by serving as a drug information resource; monitoring and managing medication regimens per protocol or per consult; and managing pharmacy inventory.III. Principal Duties and ResponsibilitiesThe following is a summary of the major functions of this individual's job. He/she may perform other duties, both major and minor, not mentioned below, and specific functions may change from time to time. The pharmacy resident is responsible for supervising and performing pharmacy operations in accordance with hospital policies and procedures, ethical and professional practices, accreditation standards, State and Federal requirements and National Patient Safety Goals regarding medication management.Until a resident is licensed as a pharmacist by the Kentucky Board of Pharmacy, all activities will be conducted under immediate supervision of a licensed pharmacist pursuant to KRS 315.020. Once licensure is obtained, all activities may be performed without immediate supervision.Interprets and reviews medication orders for correct drug, dosage, route, frequency, allergies, therapeutic duplication, interactions, contraindications, and potential adverse events for all ages servedEnsures drugs are available to patients at the time they are needed to avoid delays in therapy and promote positive patient outcomePerforms pharmaceutical calculationsPrioritizes workflow by patient impactSupervises/performs production of compounded products (sterile and nonsterile, hazardous and nonhazardous) for first doses, scheduled deliveries and batched productsProvides technician oversight and assumes role of supervisor in routine daily activitiesOversees dispensing and reconciliation of medications including controlled substances; management of expiration dates and drug recallsMaintains knowledge of local, state and federal regulations regarding pharmacy practice as well as knowledge of all applicable accrediting bodies medication management standards (e.g. the Joint Commission, HFAP, etc.)Demonstrates supervisory skills necessary to plan, organize and direct activities in the pharmacy in a cost effective mannerEffectively utilizes pharmaceutical referencesServes as a drug information expert and resource regarding medication managementHas a working knowledge of pharmacy operations including: pharmacy operating system, use of patient record, purchasing, inventory and formulary management, automation, packaging, USP 797, USP 795, USP 800, and pharmaceutical wasteInitiates, documents, and participates in department activities that promote positive patient outcomes. These activities may include, but are not limited to:Identifying medication process issues and implementing solutionsClinical and cost savings initiatives, therapeutic drug monitoring and appropriate formulary utilizationProvides comprehensive pharmacy clinical services including but not limited to: anticoagulation management/monitoring, pharmacokinetic dosing, renal dosing, nutritional support servicesParticipates in antimicrobial stewardship and opioid stewardshipParticipation in Code Blue activities as applicable to siteMedication usage evaluationsAdverse event reportingWorking with inter-professional teams to optimize order sets, automated dispensing cabinet inventory, and medication related policiesAssists in the orientation and training of technicians and studentsServes as an active member of the inter-professional team on patient care units. Participates in unit based huddles and activities if available.
