The Opportunity: Town Square Health is seeking a Manager, Population Health to support the execution of high-priority population health, operations, and technology initiatives. This role is ideal for a highly organized, execution-oriented professional who can take loosely defined problems and drive them to clear, measurable outcomes.Key ResponsibilitiesProgram & Project OwnershipOwn cross-functional initiatives from scoping through executionBuild workplans, set timelines, assign owners, and drive accountabilityProvide regular status updates to leadership on progress, risks, and decisionsCentral Operations & AutomationSupport automation initiatives across call center, referrals, medical records, billing, and medication workflowsIdentify and prioritize automation opportunities based on impact and complexityPartner with Technology, Operations, and Finance on build vs. buy evaluations and ROI analysisHelp define and track operational KPIs (e.g., turnaround time, error rates, cost per transaction)Virtual Specialist Go-Live (Example Project Area)Serve as the point person for identifying, vetting, and standing up virtual specialist partners to support patient care delivery.Assess Town Square Health's needs for virtual specialty care-including clinical use cases, patient populations, specialty coverage, and technology requirements-and recommend a prioritized shortlist of specialist vendors.Partner closely with the VP, Provider Strategy & Population Health to translate strategic vision and priorities for virtual specialty care into executable plans, while owning day-to-day management of specialist relationships.Own the specialist partner relationship end-to-end, including contract coordination, technical and operational go-live, and post-launch stabilization once patient care is live.Act as the internal owner and connector for virtual specialist partnerships, working closely with clinical, technology, and operations leaders to inform decisions around workflows, clinical hand-offs, technology integrations, and performance expectations.Population Health BuildConduct structured vendor evaluations focused on ROI and operational impactSupport vendor selection and implementation planningHelp design population health workflows and operational handoffsSupport the chronic condition management program buildsQualifications3-6 years of experience in healthcare operations, value-based care, healthcare technology, or consultingDemonstrated ability to manage projects end-to-end with minimal oversightStrong organizational, analytical, and problem-solving skillsAbility to operate effectively in ambiguous, fast-paced environmentsExcellent communication and stakeholder management skillsExperience working cross-functionally with clinical, operations, and technology teamsWhat We OfferCompetitive compensation and performance-based incentivesSalary range: $100,000 - $115,000Comprehensive benefits (medical, dental, vision, 401(k))Flexible hybrid work model (preferred in Chicago or Ann Arbor)Opportunity to help transform how healthcare is delivered to older adultsTown Square Health is an equal opportunity employer committed to building a diverse and inclusive team.
