Position Summary The primary purpose of this position is to ensure the safe and effective use of medications by dispensing prescribed medications and pharmaceuticals and by providing clinical pharmaceutical care to HCHC and community patients according to professional standards and state and federal legal requirements. Responsibilities * Performs all aspects of filling and checking prescriptions while complying with all state and federal legal aspects of the practice of pharmacy. * When alerted by a Pharmacy Technician or directly by the pharmacy software; the Pharmacist should advise technicians, prescribers and patients of potential problems to help ensure that medications are taken correctly and adverse medication reactions are less likely to occur. * Participates in resolving any medication therapy problems including therapeutic duplications, medication allergies, clinically significant drug-drug, drug-disease, drug-nutrient, or drug-laboratory test interactions. * Provides concise, applicable, comprehensive, and timely responses to requests for drug information from patients and health care providers. * Provides medication teaching upon request. * Administers immunizations based on collaborative practice agreement. * Assesses and prescribes contraception for HCHC patients based on the State of Virginia and HCHC protocol. * Oversees the implementation of and participates in clinical pharmacy services as applicable. Services may include: diabetic services, hypertension services, medication therapy management, comprehensive medications reviews and other programs as defined by HCHC. * Serves as a preceptor for Pharm.D. candidates and other students on rotation at HCHC Pharmacy. * Understands and follows the policies and procedures of HCHC and the HCHC Pharmacy. * Supervises and assists Pharmacy personnel and assigns duties in the absence of the Pharmacy Director. * Identifies language barriers of the patient and seeks out options to improve communication of health information. * Addresses customer concerns and/or directs them, as appropriate, to other staff members. * Complies with and set example of compliance with the policies/procedures/practices of HCHC and complies with legal requirements for non-profit organizations. * Complies with all state and federal regulations/law pertaining the practice of Pharmacy. * Vigilant in the performance of duties and attention to accuracy and detail, to help minimize medication error. * Will report the medication events as defined in policy and procedure. * Helps oversee inventory control procedures to assist in keeping medication stock at optimum levels for filling prescriptions as well as ensuring sound fiscal operations. * Supervises the proper filing of pharmacy records in proper chronological order. * Is responsible for receipt of controlled medications per procedure. * Assists in entry of purchases and prescriptions dispensed in the perpetual inventory spreadsheet and assists in reconciliation as necessary. * Maintains compliance with the procedures of the 340B Program by maintaining updated knowledge of the 340B Program and providing day-to-day management of the 340B Program. * Serves as a resource for 340B questions. * Active participant in pharmacy activities and makes suggestions to improve pharmacy services and operations as needed. * When requested by HCHC Administration or the Pharmacy Director, represents Pharmacy at committee or community events/meetings.
