Additional location details will be shared during the interview process.About UsThe Philadelphia Model for addressing Serious Mental Illness (SMI) in long term care provides highly structured therapeutic residential behavioral healthcare for frail adults who require skilled nursing level care and behavioral health services. Individuals served may have complex psychiatric needs and have reached maximum benefit from other community or hospital based mental health resources.Our program philosophy is centered on creating a welcoming, safe, and supportive environment where older adults with serious mental illness and complex behavioral health needs can receive carefree of stigma.A multidisciplinary team delivers integrated long-term care and behavioral health services. All team members are expected to uphold our core values of Choice, Hope, Integrity, Respect, and Professionalism while supporting diversity, equity, and inclusion.OverviewThe Behavioral Wellness Center at Girard is seeking a licensed Social Worker to support adults receiving behavioral health and mental health services in a structured, onsite treatment environment. This role focuses on clinical social work, care coordination, and interdisciplinary collaboration to support individuals with serious mental illness.Key Responsibilities• Conduct comprehensive psychosocial assessments• Participate in individualized treatment planning and care coordination• Provide clinical social work services and ongoing case management• Link clients to community resources including healthcare, benefits, housing, and social services• Maintain accurate and timely documentation in the Electronic Health Record (EHR)• Participate in interdisciplinary team meetings and clinical case reviews• Support discharge planning and continuity of care• Ensure compliance with HIPAA, confidentiality standards, and regulatory requirements• Collaborate with internal and external providers to support client stability and recoveryQualificationsEducation and Licensure• Master's degree in social work (MSW) requiredExperience and Skills• Experience working with individuals with serious mental illness strongly preferred• Experience in behavioral health, mental health, long term care, or human services settings• Strong knowledge of community resources and social service systems• Excellent clinical documentation and organizational skills• Ability to manage a caseload in a fast paced, fully onsite environment• Proficiency with Electronic Health Records (EHR)Why Join Us• Competitive starting pay• Mission driven behavioral health organization• Collaborative, multidisciplinary clinical team• Opportunity to support a specialized population with complex needs• Meaningful, impactful work in the Philadelphia communityPre Employment and Workplace PolicyEmployment is contingent upon the successful completion of all pre-employment requirements, including a physical examination, background check, verification of employment, and drug screening.
