Job Description

St. Croix Health is seeking a dedicated and knowledgeable Athletic Trainer/Orthopedic Navigator to join our healthcare team. This is a full-time (1.0) position that will focus on blending traditional athletic training responsibilities with the role of an orthopedic navigator. You will play a crucial role in providing comprehensive care and support to athletes, particularly those involved in the Unity School District programs, and support the broader orthopedic practice by coordinating care for our orthopedic patients. Athletic Trainer:1. Perform assessments of athletic injuries or illnesses accurately and utilize appropriate referral patterns.Bi-weekly injury log to sports med physician and manager2. Collaborate with providers to accurately assess diagnosis, succeed in implementation of rehabilitation plan, and provide optimal patient experience.Bi-weekly injury log to sports med physician and manager3. Communicate effectively to necessary team members (physicians, staff, and coaches) and athletes' parents.4. Rehabilitate and progress athletic injuries/illnesses appropriately with proper supporting documentation.5. Provide exceptional emergency care and on-field evaluations.Participate in designated outreach activities, including covering school contracts and events, and engage in sports medicine outreach outside the clinic setting. Orthopedic Navigator:1. Navigate patient care throughout health systemOversee the planning, scheduling and implementation of pre-operative and post-operative appointments and procedures for navigated patientsServe as the central point of contact for navigated patients, including interactions with Primary Care, Ortho, Rehab, Preop Education RNManage surgery schedule and coordinate with surgery scheduler to finalize scheduleCollect and report data from patients on barriers, health care, medications and prior treatments2. Provide direct patient care, education, and follow-upManage ortho referral work quesContacting patients at designated intervals to act as an informational resourceProvide education materials Manage Ortho phone calls and messagesRequirementsEducation & Licensure: Bachelors degree with Athletic Training certification from the BOC (Board of Certification specific for Athletic Trainers)BLS Card (Must be issued from the American Heart Association or American Red Cross for Healthcare Providers)Current Athletic Training WI licensure and/or Registered as an Athletic Trainer in Wisconsin, or ability to obtain Experience: 2+ years preferred but not requiredMember of NATA (National Athletic Trainers' Association) preferredCompetencies Knowledge, Skills & Abilities: Ability to maintain highly confidential informationAbility to analyze and solve problemsAbility to develop and maintain recordkeeping systems and proceduresExcellent critical thinking, reasoning and analytical skillsGeneral knowledge of healthcare service deliveryAbility to work in a fast-paced environmentAbility to work productively with minimal supervision and is self-motivatedAbility to follow written and verbal directions/instructionsAbility to create, to compose, and edit written materials and communicate effectivelyAbility to deal effectively and assertively with a broad range of people under varying pressure situationsSkill in organizing resources and establishing prioritiesPhysical Requirements:Must be able to lift to 50 pounds, and frequently lift or carry objectsProlonged periods of standing, bending and reaching St. Croix Health is an Equal Opportunity Employer.
