Job Description

The Substance Use Disorder (SUD) Treatment Program assumes responsibility for the provision of substance use disorder services to veterans who are admitted for outpatient SUD treatment. The services will be provided primarily in outpatient setting and may provide some services to Residential Rehabilitation patients, as needed. The services will be provided primarily in outpatient setting and may provide some services to Residential Rehabilitation patients, as needed. Duties of this position are, but not limited to: * Conduct psychosocial assessments to determine needs of veterans and family members. * Conduct intake substance use disorder assessments for patients to determine appropriate VA program needs. * Active participant in the treatment planning process with other disciplines. * Participate in transition/discharge planning with other disciplines and will be responsible for ensuring that transitions plans are executed in a manner that is timely and appropriate. * Provide individual, family and group counseling as appropriate. Individual and group services will be focused on substance use disorder treatment. * Provide consultation/education to veterans and families on community resources, advance directives and VA benefits. * Act as a consultant to other team/staff members. * Provide consultation/education to students of all disciplines on clinical practice and VA policy and procedure. * Provide case management to veterans and families, as needed, throughout the continuum of care. * Provide a full range of social services within commonly accepted standards of their profession, which includes case work, individual, family and group counseling, discharge planning, case management and other services as appropriate. * Provide wellness/prevention education and facilitate patient/family support groups as appropriate. * Coordinate treatment and communicate with the care lines to facilitate continuity of care for veterans. * Other duties as assigned Work Schedule: Monday-Friday; 8:00am-4:30pm, and 1 evening a week Telework: Ad-hoc may be available Virtual: This is not a virtual position.
