Job Description

Title: Physician (Street Medicine) Department: Greater New Haven Healthcare for the Homeless Reports to: Clinical Director Location: New Haven County, Connecticut Position Summary: The Per Diem Street Medicine Physician provides reasonable and prudent medical/surgical care. They will practice appropriate management, using a harm reduction trauma informed approach with sound clinical judgment based on best practice standards and evidenced based medicine. Provides general medical care, patient education and treatment to patients of CS-HHC. Promotes and maintains health by providing services under the supervision of the Clinical Director of Homeless Care and the Chief of Medicine. Essential Duties and Responsabilites: * Maintains confidentiality in regard to patients, staff and program matters at all times. * Consults with other staff or specialists to make appropriate referrals for acute, chronic and preventive health care, as may be indicated. * Coordinates patient care with colleagues, nursing, front desk outreach, case management staff and other agencies as necessary. * Educates patients on preventive health care and chronic disease management. * Treats patients and staff respectfully, courteously, appropriately. * Maintains a safe and clean working environment by complying with procedures, rules and regulations. * Protects clients and employees by adhering to infection-control policies and protocols. * Completes the charting of notes, forms, and coding in a timely and efficient manner. Enters appropriate legible notations, documents the encounters appropriately. Qualifications: Education: Degree from an accredited Medical School. Documented completion of Residency training from an accredited institution. CPR Certification is required. Experience: Board Certification or Board Eligibility. (Board certification must be attained within the timeframe dictated by one's specialty in order to remain on staff at the CS-HHC if one was originally board-eligible at the time of one's hire.) Connecticut Licensure. Admitting privileges at Yale-New Haven Hospital, HSR, and/or Griffin Hospital as indicated.
