Ditmas Children's Nursing & Rehab - Pediatric Social Services Professional Supporting Children. Strengthening Families. Ensuring Safe Transitions. We are seeking a dedicated and compassionate professional to provide psychosocial support and ensure the safe transition of care to home for every child admitted to our pediatric facility. This position plays a vital role in meeting both the medical and psychosocial needs of pediatric residents during their stay and after discharge. In addition to direct service, this role includes administrative responsibilities related to the planning, organization, and implementation of Social Services policies and procedures in accordance with federal, state, and local standards and regulations. Responsibilities & Duties Participate in the development, review, and implementation of departmental policies and procedures. Conduct resident assessments and psychosocial screenings. Initiate, implement, and evaluate individualized care plans. Provide resident and family communication, education, and discharge planning. Facilitate access to healthcare services, medical resources, insurance coverage, community resources, and public assistance programs. Maintain effective relationships and communication with interdisciplinary team members and facility stakeholders. Collaborate closely with other departments and services, including the dialysis department. Provide comprehensive social services to pediatric residents. Maintain knowledge of relevant services, agencies, and regulations related to pediatric healthcare, including Child Protective Services and child abuse prevention. Participate in facility surveys and inspections as required. Qualifications & Skills Bachelor’s or Master’s degree in Social Work, Sociology, or Special Education (required). Strong communication and organizational skills. Knowledge of pediatric healthcare regulations and community resources. Bilingual candidates are strongly encouraged to apply.
