A travel Pharmacist opportunity is available with a start date of March 17th for a 20-week assignment in Carbondale, CO. This role offers the chance to support a facility transitioning its electronic medical records system from Meditech to Epic, requiring solid Epic experience and knowledge of Meditech a plus. The position involves rotating shifts working 40 hours per week with early morning start times and rotating weekend coverage. Key Responsibilities: Prepare and dispense medication orders accurately as per physician requests. Maintain medication stock in patient care areas to ensure availability. Ensure safe, appropriate, and cost-effective medication therapies in adherence to policies and protocols. Interpret medication orders and maintain computerized patient medication profiles. Compound and dispense pharmaceuticals, including IV admixtures, chemotherapy, and parenteral nutrition products. Comply with all federal, state, and local laws regarding medication handling, including controlled substances. Monitor drug therapies for interactions, contraindications, allergies, and appropriate dosing. Conduct pharmacokinetic drug dosing and review culture and sensitivity data to optimize antibiotic use. Report adverse drug reactions and support formulary management through therapeutic substitutions. Provide clinical consultation and drug information to medical staff, patients, and families. Document clinical activities and participate in quality improvement initiatives. Participate in staff training and serve as a member of the Code Response Team. Desired Qualifications: Active Pharmacy License in Colorado in good standing. Graduate of an ACPE-accredited School of Pharmacy with a BS Pharmacy degree. BLS and ACLS certification from the American Heart Association. Flu vaccine compliance is mandatory during flu season (Oct 1 - Apr 1). Previous experience with Epic electronic medical records system. Meditech training advantageous. Schedule: Rotating days/evenings, 40 hours per week. Start times at 6:30 AM or 8:00 AM from Monday to Friday. Rotating weekend shifts from 7:00 AM to 12:00 PM.
