The HCA officestaff are responsible for ensuring participant care is coordinated and managed appropriately according tostate and federal guidelines. The HCA Supervisor plans, implements and evaluates patient's plan of carefor appropriateness according to the individual patient's needs. Reports directly to the HCA Director.The HCA Supervisor acts as the HCA Director, in the absence of the HCA Director.SpecificationsEducation: RN DegreeExperience: 1-2 years working as a Nursing Supervisor, with at least 1 years' experience working withthe frail elderly population. Home Care experience preferred.Number and Type of Employees Supervised (optional): 20+Licensure, Registry or Certification Required: Currently listed as an RN in the state of North Carolina.Must have no substantiated findings of neglect, abuse, or misappropriation of patients. Valid drivinglicense with personal vehicle insurance required. A dependable means of transportation is mandatory.Special Training: Good driving record; able to communicate effectively both verbally and in writtenform; knowledge of and care of adults with chronic health conditions. Only act within the scope of his orher authority to practice. The HCA Supervisor plans, implements and evaluates patient's plan of care forappropriateness according to the individual patient's needs.Immunizations: Be medically cleared for communicable diseases and have all immunizations up to datebefore engaging in direct participant contactAges of Patients Rendered Care: Adult & Geriatric Key Responsibilities: (*denotes an age-related skill or task):• Accomplishes nursing human resource objectives by selecting, orienting, training, assigning,scheduling, coaching, counseling, and disciplining employees; communicating job expectations;planning, monitoring, appraising job contributions; recommending compensation actions; adhering topolicies and procedures.• Meets nursing operational standards by contributing information to strategic plans and reviews;implementing production, productivity, quality, and customer-service standards; resolving problems;and identifying system improvements with the HCA Director• Meets nursing financial standards by providing annual budget information; monitoring expenditures;identifying variances; and implementing corrective actions with direction from the HCA Director• Maintains home care guidelines by writing and updating policies and procedures.Last Updated: September 4, 2020• Assures quality of care by developing and interpreting nursing division's philosophies and standardsof care and enforcing adherence to state regulations• Provides information to patients and health care team by answering questions and requests.• Resolves patient needs by utilizing Home Care Coordinators and team strategies.• Maintains safe and clean working environment by designing and implementing procedures, rules, andregulations, calling for assistance from other health care professionals as needed.• Maintains documentation of patient care services by auditing participants' and department records.• Maintains professional and technical knowledge by attending educational workshops and reviewingprofessional publications• Maintains a cooperative relationship among health care teams by communicating information;responding to requests; building rapport; and participating in team problem-solving methods.• Accomplishes organizational goals by accepting ownership for accomplishing new and differentrequests and exploring opportunities to add value to job accomplishments.• Completes all initial assessments, re-assessments and supervisory visits every 90 days.• On call evenings and after hours and available during the times staff is in the homes providing care.• Other duties as assigned by HCA Director
