We are dedicated to providing exceptional patient care in the healing of body, mind, and spirit. **Come be part of the Excellence.** **Basic Job Function Summary** The Home Care Liaison is responsible for refining and improving the transition processes across all sites of home and hospice care. This role manages the efficient and effective facilitation of referrals to the post‑acute homecare/hospice agency, educates acute care staff (including physicians) about available homecare/hospice services, and may participate in business development activities related to growth or new program expansion. **Minimal Qualifications** + RN- LPN- MSW, OT, PT or SLP + Excellent communication and customer service skills. + Strong critical thinking abilities. + Knowledge of Medicare rules and regulations. + Working knowledge of managed care environments. + Experience multitasking across multiple computer systems. + Current registration or licensure in the state of Illinois preferred. + **3-4 years** of professional clinical practice in an acute care environment preferred. + **1-2 years** of direct patient care experience in a home care setting preferred. + **1-2 years** intake or sales experience preferred. + Valid driver's license and reliable transportation to/from work sites. + Ability to consistently demonstrate commitment to the mission, Organizational Code of Ethics, and Compliance Program. **Position Highlights and Benefits** + Medical, dental, and vision insurance - **day one benefits** + Short‑ and long‑term disability + 403(b) with matching contribution + Generous paid time off **plus 7 paid holidays** + Comprehensive orientation + Tuition reimbursement up to **$5,250 per year** + Daily Pay option + **Pay Range:** $29.72 - $47.55 **Ministry/Facility Information** **Loyola Medicine Home Care and Hospice** is a member of **Trinity Health At Home** , a national home care, palliative care, and hospice organization serving communities across nine states.
