The Medical Records Associate ensures that a complete medical records program is maintained in accordance with company standards, federal, state, and local standards, guidelines, and regulations. Essential Activities and Tasks Administration and Reporting - 50% * Reviews all records on the nursing units. * Assists in developing procedures to ensure records are properly assembled, coded, signed, indexed, etc. before filing. * Provides clerical support for the nursing management team. Operations Management - 30% * Assists in planning, developing, organizing, implementing, evaluating, and directing the medical records in accordance with established policies and procedures. * Collects, scans, and attaches resident and patient information to the electronic medical record (EMR). * Ensures that incomplete EMRs are returned to the Nursing Department for completion. * Gathers and organizes information for insurance appeals. * May be responsible for assigning ICD-10 diagnosis codes. * Keeps schedule, gathers and organizes information for insurance re-certifications as assigned May initiate, prepare, and ensure completion of all Medicare certifications and re-certifications. * May notify residents, patients, and families of non-coverage of all Medicare or insurance recipients. * Completes five-day admission audit as assigned. * Audits EMR for completeness and reports finding to Director of Nursing (DON), Administrator, or Unit Manager. * Gathers documentation needed for federal, state, and city surveys. * Attends all-employee and departmental meetings and in-service training as scheduled or as directed. Quality and Compliance Management - 20% * Assists in establishing procedures to be followed in the collection of electronic medical records. * Advises DON on federal and state laws concerning medical records. * Ensures established policies and procedures for the electronic medical records are followed by all staff. * Reviews department policies and procedures, at least annually, and participates in making recommended changes. * Collects and audits physician signatures notifying the DON or Unit Manager when unable to obtain. * Maintains the confidentiality of resident and patient information. All other duties as assigned. Qualifications Education * High school diploma or equivalent required. * Associate degree in health information or related field preferred. Experience * Two years experience in medical records in a long-term care, hospital, or other related healthcare setting required. * Must have working knowledge of diagnosis codes. Other Requirements * Must be able to read, write, speak, and understand the English language.
