Responsibilities Glen Oaks Hospital is a 54-bed private mental health hospital in a relaxed setting in Greenville, Texas. The Step-Down Navigator is responsible for implementing a revised process of meeting patient needs for a continuum of care. They will meet with all eligible patients to discuss the benefits of Outpatient services as part of their continuum of care treatment. The Step-Down Navigator will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to Outpatient eligibility, into Midas for trending report purposes. They will have a thorough understanding of the Step-Down Playbook, implementing components of the Playbook as needed. It is expected that the Step-Down Navigator will identify implementation barriers and evaluate the new process with the facility leaders using the data. A thorough understanding of the Inpatient workflow process, as it pertains to case management and discharge planning, is preferred. Hours: Day shift, Monday through Friday, 8am to 5pm. Qualifications Education: Bachelor's degree in mental health or behavioral sciences field, preferred. Experience: Two years minimum experience in psychiatric / mental health field, preferred. Licensure: Not required. Additional Requirements: CPR BLS certification and successful completion of Handle with Care (HWC) and Verbal De-escalation (VDE) training.
