We are currently seeking a full-time Quality Care Specialist in Sumterville, Florida to join our team. We are a non-profit Federally Qualified Health Center located in Central Florida. Appointment Scheduling: * Manage patient appointment scheduling, ensuring appointments are made accurately and timely. * Confirm appointments and contact patients with reminders for appointment, minimizing no-show rates. * Reschedule appointments and handle cancellations while maintaining efficient clinic operations. * Coordinate with provide leadership and other healthcare staff to ensure proper scheduling * Answer patient inquiries regarding appointment availability, office procedures, and healthcare services. * Provide clear, empathetic, and informative communication to patients throughout the scheduling process. * Ensure all necessary patient information is collected and entered into the system prior to the appointment. * Accurately input and update patient information in the electronic health record (EHR) system. * Maintain and track appointment records to ensure all patient encounters are documented. * Provide a high level of service by addressing patient concerns and ensuring their needs are met in a timely manner. * Assist with follow-up calls and appointment reminders to ensure patients attend scheduled visits. * Collaborate with the front desk and billing team to ensure proper insurance information is collected and appointments are confirmed for billing purposes. * Other job duties as appropriate and assigned by supervisor. Qualifications: * Certification or Registration as Medical Assistant; licensure as Practical Nurse or Registered Nurse * Experience in chronic care management, HEDIS compliance, or quality improvement programs within a healthcare setting, particularly in a Federally Qualified Health Center (FQHC) or similar community health setting. * Strong knowledge of HEDIS measures, National Quality Forum (NQF) quality measures, and healthcare data reporting requirements. * Knowledge of Electronic Health Record (EHR) systems and data entry. * Excellent analytical skills with the ability to analyze data, identify trends, and implement corrective actions. * Strong communication and interpersonal skills, with the ability to work effectively with clinical teams, patients, and external partners. * Ability to maintain confidentiality and comply with HIPAA and other regulatory guidelines. * Proficiency in Microsoft Office Suite (Excel, Word, PowerPoint).
