Job Description

We are seeking a dedicated Hospice Field Registered Nurse (RN) to provide compassionate, patient"‘centered care across central and southeastern Illinois. This role supports patients and families during one of life's most vulnerable transitions while working within a collaborative interdisciplinary team. Location: Champaign, Vermilion, Edgar, Douglas, Moultrie, and Macon counties. Schedule: 8:00"¯AM - 4:30"¯PM, Monday-Friday (limited on"‘call and weekend duties). Responsibilities Coordinate and provide direct and indirect patient care using the nursing process to meet physical, psychosocial, environmental, and spiritual needs. Perform case"‘management activities"”assessment, coordination, planning, monitoring, implementation, and evaluation. Document and transmit admission, routine, supervisory, and discharge notes in accordance with policy. Ensure timely entry and transmission of physician orders, verbal orders, and care"‘plan updates. Supervise and assign home"‘health aide duties, providing written instructions and oversight. Collaborate with clinicians, physicians, care coordinators, insurers, and other disciplines through case conferencing. Diagnose and assess patients for hospice placement per Medicare guidelines; conduct home visits to monitor symptoms and provide palliative interventions. Handle triage phone calls, conduct post"‘mortem care, notify County Coroner, funeral homes, and physicians as required. Offer comfort to patients and families and refer to Bereavement Coordinator when appropriate. Provide care according to the plan of care and orders, directing the licensed practical nurse as needed. Qualifications Licensed Registered Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR). Minimum of 2"¯years of nursing experience. Demonstrated understanding of hospice regulatory requirements, documentation standards, and recertification processes. Valid auto insurance (varies by location). College diploma in nursing.
