We are looking for a full-time Wraparound Facilitator to serve families who have adopted children/youth out of the foster care system, and who are experiencing challenges with the child's/youth's behavior and mental health. The Wraparound Facilitator oversees the post-adoption wraparound process for up to five families, including initiating engagement, forming teams, developing safety and care plans with a strength-based approach. There is flexibility in the schedule to promote work/life balance. This position requires significant in-person presence and duties, but there is also the option for one to two days per week of remote work at home after initial training.Key Responsibilities:Under the leadership of the Wraparound Supervisor, you will provide direct services to families and youth, including:Initiates the engagement process by understanding the family's history and current situation through their stories, identifying their needs, strengths, trauma, and key relationshipsExplains the post-adoption wraparound process to the family, covering its values and principles, emphasizing the significance of the family's input, choices, and preferencesAssists parents and youth in identifying and inviting potential Child and Family Team (CFT) members, orienting them to the wraparound process and their roles, and facilitates the CFT meetingDevelops and facilitates individualized, strength-based, and culturally sensitive care plans with individualized strategies that include supportive linkagesProvide direct support in homes and communities, addressing the unique needs of youth and familiesCoordinate with team members, community resources, and educational services to ensure comprehensive supportMaintain meticulous records, ensuring all interventions and plans are well-documentedQualifications:Bachelor's degree in a behavioral science field preferred2+ years of case management experience preferredAt least one year of experience working with high-risk youth or families, or comparable lived experience in foster care or adoption settingsExceptional interpersonal skills, with a commitment to culturally competent careExperience in child welfare, juvenile justice, and crisis interventionStrong commitment to cultural competency and ethical practicesValid driver's license and reliable transportation are requiredJoin Our Team:At Koinonia Family Services, we are committed to creating a supportive, mission-driven workplace where employees feel valued and connected to the work they do. Facilitators are responsible for tasks such as visiting client homes, transporting youth, engaging with community partners, often in locations not easily accessible by alternative transportation methods. The nature of these responsibilities necessitates the ability to drive to ensure timely, reliable, confidential and effective support for the youth and families we serve.
