Serves as co-leader in an interdisciplinary approach to patient groups. Deliver patient-centered physical therapy interventions in a home health setting, adjusting plans of care as patients progress. Document patient progress, modify goals, and contribute to care coordination with the interprofessional team. Educate patients and families on home exercise programs, safety with activities of daily living, and mobility strategies to support independence. Requirements Availability for a day shift schedule (8 hours) on a travel assignment with 40 hours per week. Willingness to work in a home health - hospice setting. Strong collaboration skills to work within an interdisciplinary team. Ability to document and communicate patient progress effectively and maintain appropriate care records. Adaptability to a travel assignment and to providing care in patients' homes.
