Registered Dietician Nutritionist $5,000 HIRING BONUS Posted on March 2, 2026 Locations Eckert, CO Montrose, CO Sheridan, WY Salt Lake City, UT Phoenix, AZ Colorado Springs, CO Denver, CO Description We are looking for a Registered Dietician (RD/RDN) with an active state license or eligibility for licensure to join our Senior CommUnity Care team. Position: Senior Community Care of Colorado PACE (Program of All-Inclusive Care of the Elderly) Location: Eckert, Colorado (relocation bonus offered) Salary: $64,000 - $80,000 annually, based on experience Schedule: Monday - Friday, 8 a.m. - 5 p.m.; travel to participants' homes may be required. Plan and implement nutrition interventions based on data. Communicate findings to the Interdisciplinary Team. Provide nutrition education to participants and caregivers. Coordinate meal serving, monitoring kitchen standards, and contracted meal services. Provide education and supervision to the Dietary Aide. Qualifications Bachelor's or advanced degree in food and nutrition or dietetics. Minimum one year of experience working with frail or elderly populations. Valid driver's license and reliable transportation. Medical clearance for communicable diseases and up-to-date immunizations. Ability to clear Background Check and Drug Screen. Skills & Knowledge Understanding of the physical, mental and social needs of frail older adults. Effective assessment of nutritional needs. Excellent written and oral communication skills. Ability to work within an Interdisciplinary Team independently or collaboratively. Strong organizational skills. Computer literacy for timekeeping, in"‘servicing, and documentation. Essential Functions Deliver quality Nutrition Care through the Academy of Nutrition and Dietetics framework. Conduct annual and bi"‘annual re"‘assessments as per the Plan of Care. Participate in Interdisciplinary Team meetings, 24"‘hour care coordination. Monitor weight, medical diagnosis, drug-nutrient interactions, hydration, intake, skin breakdown, and report changes. Document nutrition notes and other activities in the medical record. Provide nutrition counseling and education to participants and caregivers. Provide nutrition educational programs to other staff. Assist in meal monitoring, intake, diet compliance, and tray accuracy. Oversee dining program operations and maintain food inventory. Plan and manage special events with meal service logistics. Develop therapeutic dining programs and dietary census. Supervise Dietary Aide: initial and annual competencies, performance evaluation. Ensure direct reports' responsibilities and accountability are clear. Maintain kitchen cleanliness, portion control, therapeutic diet compliance, food purchasing and inventory. Manage therapeutic diets per physician orders. Act as liaison with contracted meal service to meet participant needs. Assist in policy and procedure development for nutrition and dining. Protect privacy and confidentiality of employee and participant information. Participate in quality improvement initiatives. Perform other duties and committee participation as requested.
