Hybrid option opens after a 90"‘day probation/training period. Option is 2 days a week from home, 3 days onsite (days coordinated with direct management). Role Description The purpose of a Medical Scribe at Oak Street Health is to support our primary"‘care providers with clinical documentation so that they can focus on providing exceptional care to patients. Scribes assist providers throughout the patient care journey"”joining them in the exam room to observe and document, and following up after visits. Beyond the typical scribe role, these team members serve as clinical documentation assistants to their paired provider. Internally we call them CISs (Clinic Informatics Specialists) in recognition of their role in supporting accurate, specific, and timely documentation. Scribes become experts in our value"‘based care model, chronic"‘condition documentation, ICD"‘10 and CPT coding, and the use of population"‘health data streams.
