Summary This position is located in the Veterans Health Administration (VHA), Southeast Network (VISN 7), VA Augusta Health Care System (VAAHCS) in Augusta, GA located in the Mental Health Service. The Social Worker provides clinical services within the Mental Health Service, supporting collaboration between the Substance Use Disorder (SUD) treatment program and community-based Supportive Services for Veteran Families (SSVF) providers. Responsibilities VA Careers - Social Work: VA Careers - Licensed Clinical Social Worker: Rewards of a Allied Health Professional GS-11 Functions: Independently conducts comprehensive biopsychosocial assessments for Veterans with substance use disorders and housing instability. Provides individual and group interventions using evidence-based, recovery-oriented practices to promote sobriety, stability, and community reintegration. Develops and implements individualized treatment and housing stabilization plans in collaboration with SUD providers, SSVF case managers, and Veterans. Serves as the primary liaison between VA SUD services and community SSVF agencies, ensuring coordination of care, timely referrals, and consistent communication. Participates in multidisciplinary case conferences, community partner meetings, and Homeless Program coordination efforts. Provides consultation to SSVF and VA staff on substance use, relapse prevention, and behavioral health risk management. Facilitates transitions of care for Veterans moving between VA programs, SSVF placements, and community housing. Conducts risk assessments, crisis interventions, and safety planning as clinically indicated. Documents all contacts and coordination activities in accordance with VA, Joint Commission, and CARF standards. Engages in quality improvement, data monitoring, and performance initiatives to strengthen integration between SUD and SSVF services.
