Description Job Title: Pharmacy Technician Department: Greater Rochester Independent Practice Association (GRIPA) Location: 100 Kings Highway South, Rochester, NY Hours Per Week: 40 hours Schedule: Full-time, Days This position will be employed by the Greater Rochester Independent Practice Association, INC. (GRIPA). GRIPA was founded in 1996 as a unique collaboration between hospitals and physicians, for the purpose of simultaneously improving the quality and efficiency of health care. This position will not be employed by Rochester Regional Health (RRH). SUMMARY The Pharmacy Technician is responsible for working hand-in-hand with a pharmacist to assist in improving quality of care, reducing costs, providing medication assistance related to medication adherence, completing medication histories, participating in follow-up calls to patients/providers, and assisting on projects related to improving the medication use process. Requires collaboration with providers, care managers, other entities (insurance company staff, home health care providers, pharmacies) and other health care professionals at various sites of services. RESPONSIBILITIES Clinical Support Reduce Pharmacy Costs: With the assistance of a pharmacist, reduce pharmacy costs on identified high cost medications Support outcome reporting related to medication cost initiatives Follow-up calls: Participate in follow-up calls to patients and healthcare providers Follow-up calls to healthcare providers to obtain outcomes related to pharmacist's interventions Follow-up calls to patient to ensure medication changes have been implemented and to obtain objective information (blood glucose and BP values) for further optimization of medications by pharmacists Medication Adherence: Provide support for medication adherence by identifying barriers to adherence, assisting with improving medication affordability, assisting with medication reminders, linking to medication counseling, etc. Completion of medication financial assistance applications (i.e. EPIC, PAPs, etc.), as needed Medication Histories: Complete medication histories (in-home, telephonic) with handoff to another member of the care management team, as appropriate Medication-use Projects: Participate in projects related to the medication-use process with goals of improving quality of care and reducing costs Learn and gain proficiency in applications and resources used in high cost drug and care management programs Coordinate and collaborate with contracted entities including insurers, employer groups as well as key health system entities to drive positive outcomes Employer contract initiatives: Participation in employer contract specific initiatives in collaboration with the care management team Community resources: Knowledge of community resources available for medication management and ability to direct patients to participate in use of these resources Perform other duties as assigned to support business needs including maintaining and updating GRIPA resources Quality and Outcomes Engage in performance improvement activities to promote quality care Adherence to ethical, legal and practice standards of care management practice Self-monitoring of quality metrics to promote positive population health outcome Documentation and Reporting Ability to use the EPIC EMR and other documentation software to manage the population and promote care team communication Ensures timely and complete documentation following any patient and/or provider related contact per department documentation policy/procedure Reports on successes as well as areas where there are opportunities of improvement Professional Development/Education Demonstrates ability to perform day
