JOB REQUIREMENTS: Job Description: Position Summary: Personal CareWorker provides personal care services to consumer(s) of IndependenceFirst. Personal Care Worker is directly supervised by the RegisteredNurse Care Coordinator. Essential Functions of the Job Assistingconsumer with getting in and out of bed. Assisting consumer withtoileting/incontinent care Assisting consumer with bathing or showeringAssistance with eating. Assistance with grooming to include mouth,denture, hair, shaving and nail care. Assistance with dressing andundressing. Care of eyeglasses and hearing aids. Assistance withmobility and ambulation including use of gait belt, walker or cane.Simple transfers including bed to chair or wheel chair and reverse. Skincare excluding wound care. Meal preparation and serving. Light cleaningin areas used during personal care activities. Changing of consumer'sbed, washing bed linens and consumer's personal clothing. AdditionalResponsibilities Completion in a thorough, accurate and timely manner ofall time sheets and documentation of consumer's condition as necessary.Keep consumer's information confidential. The above listed essentialfunctions and qualifications are not all-inclusive as changes and/orother responsibilities may be assigned as necessary. Experience andSkills: The ideal Pool PCW will have total care experience and will becomfortable assisting consumers with diverse needs. All PCWs must haveadequate training in the provision of personal care services and in eachskill that the PCW is assigned. Documentation of training ordemonstrated skill is required. Previous experience may be accepted inlieu of formal training. Hands-on experience with a variety ofdurable medical equipment, including but not limited to: Hoyer lifts,Gait belts, Transfer boards, etc. Previous experience with total careconsumers Excellent communication skills and ability to relate wellwith others Set availability, weekend/evening availability preferred Good problem solving skills A working phone, with voicemail thatyou check and respond to regularly Be able to pass a CaregiverBackground check and have no criminal convictions that are significantlyrelated to the job Have ability to travel to areas of Milwaukee,Ozaukee, Waukesha, and Washington Counties Dependable and ReliableIndependenceFirst is a EOE/M/W/Vets/Disability Employer.
