Employment Type: Full time Shift: Description: Trinity Health LIFE New Jersey, a member of Trinity Health PACE, is seeking a Community Liaison for its location in Pennsauken, NJ. The Program of All-Inclusive Care for the Elderly (PACE) model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in the community whenever possible. PACE serves individuals who are age 55 or older, certified by their state to need nursing home care, able to live safely in the community at the time of enrollment and live in a PACE service area. While all PACE participants must be certified to need nursing home care to enroll in PACE, only about 7 percent of PACE participants nationally reside in a nursing home. If a PACE enrollee needs nursing home care, the PACE program pays for it and continues to coordinate the enrollee's care. We encourage fellowship and independence while serving seniors and their families with dignity and respect. Delivering all needed medical and supportive services, PACE is able to provide the entire continuum of care and services to seniors with chronic needs while maintaining their independence in their home for as long as possible. _JOB Description_ The Community Liaison is responsible for developing relationships with community referral sources, agencies, health care professionals and families to provide greater understanding of Trinity Health LIFE New Jersey. The Liaison will also work closely with Trinity Health LIFE New Jersey staff to promote greater understanding of community needs. The Community Liaison is responsible for obtaining referrals for people qualified and interested in the Trinity Health LIFE New Jersey program. General responsibilities will include: Provide excellent customer service in a timely manner and deliver a marketing message that is consistent with sales and marketing strategy. Establish and maintain positive working relationships with referral sources to regularly discuss possible program candidates and to ensure a clear understanding of Trinity Health LIFE New Jersey as a vital community resource. Targeted referral sources include physicians, hospital/skilled nursing facilities, discharge planners, case managers, social workers, senior housing buildings and community organizations. Strategies will include visits at regular intervals as well as cold calls to new prospective referral sources. Collaborate with Enrollment Specialists to insure optimal referral handoff, including screening for medical & financial eligibility when necessary, to enable Enrollment Specialists to successfully convert a lead into a home intake visit and enrollment. Support all members of Enrollment team as needed to meet monthly enrollment goals, including stepping into their roles for screening, intakes, home visits, Medicaid applications, and enrollment agreement signings. Effectively provide outreach services through in-service education programs, presentations, health fairs, networking/collaborative groups, and more. Prepare appropriate marketing and enrollment collateral for department use. Assist with marketing activities as required. coordinate all tasks related to monthly New Participant Orientation. Collaborate with IDT team members to develop and offer educational/clinical presentations to referral sources and community groups to demonstrate community commitment and to increase program exposure. Submit, in a timely manner, written documentation in required Community Liaison weekly report and Enrollment CRM. Be accountable to obtain qualified referrals to meet target enrollment goals each month, quarter and for the entire year. Candidates will have: + Education: Bachelor's degree in related field preferred; HS school diploma or GED required. + Two (2) years' experience in case management, marketing or sales, preferably in a healthcare or similar environment. + Ability to function with minimal supervision and ability to work in a multicultural, multi-ethnic environment.
