Network ESC - Harlem Based Rehabilitation Facility is in need of a LMSW for their Day time Adult population.5 Days a week Schedule 9am-5pm.Must Have experience with HIV Clients Client Assessment and Treatment Planning:Conducting Assessments:LMSWs assess clients' needs, strengths, and challenges to understand their situations and develop effective interventions.Developing Treatment Plans:Based on assessments, LMSWs create individualized treatment plans that outline goals, strategies, and interventions to address clients' needs.Providing Therapy:LMSWs provide counseling, therapy, and support to clients in various settings, including individual, group, and family therapy. Case Management and Coordination:Case Management:LMSWs manage client cases, coordinating services, making referrals, and ensuring clients receive the support they need. Interdisciplinary Team Collaboration:LMSWs work collaboratively with other professionals, such as doctors, therapists, and case managers, to provide comprehensive care. Advocacy:LMSWs advocate for clients' rights and needs, working to ensure they receive appropriate services and support. Record Keeping and Documentation:Maintaining Client Records:LMSWs maintain accurate and confidential client records, documenting progress towards treatment goals.
