Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Responsibilities Deliver care to patients in a respectful non-judgmental manner and according to Patient Care Plan. Demonstrate understanding of age - and population - specific characteristics that affect the health care needs of the veteran. Perform aspects of admission /discharge procedures as directed. Collect patient data including observations of patient status.Accurately report patient/client information. Recognize subtle and significant changes in the patient in response to nursing care. Promptly recognizes urgent or emergent patient care situations, seeks assistance, and effectively intervenes when appropriate. Communicate changes in patient condition/response to nursing care. Organize and prioritize routine and non-routine nursing care activities. Provide care to patients/clients appropriate to their cognitive, physical, emotional, and chronological maturation. Accurately document and retrieve patient/client clinical information using electronic medical record. Provide effective education to patients and family members in relation to common disease processes, medications, and prescribed diagnostic and treatment regimes. Adhere to standards of practice and safe technique in preparation, administration, and documentation of medicine, in accordance with the Bar Code Medication Administration (BCMA) standards. Observe patient for adverse effects from medication and reports to the team Registered Nurse (RN). Perform other duties as assigned. Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. Mechanical inpatient lifts are provided. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Work Conditions: A Licensed practical Nurse (LPN) works in a primary care Department of Veterans Affairs (VA) setting primarily works in an outpatient, office clinic environment, caring for a range of veterans by performing direct patient care, medication administration, and clinical support for providers. The environment is typically well-lit and ventilated but can be stressful, requiring multitasking and the ability to handle several tasks at once.
