ESSENTIAL DUTIES u00b7 Assist clients and their children in accessing medical care, housing, education, public aid and job opportunities within available community resources. u00b7 Conducts home visits to gather data related to health and environmental conditions. u00b7 Follows up with clients, nurses and other service providers to determine whether clients are receiving services in accordance with the client care plan developed by the Public Health Nurse II. u00b7 Informs the Public Health Nurse II of situations that may require a reassessment of the client's health and social service needs. u00b7 Coordinates the provision of services with departmental staff, local health and social service agencies and community groups. u00b7 Maintains client records u00b7 Provides information on the program to community groups and agencies. u00b7 Maintains lists of health and social service providers in the community. _Additional duties may be required for this position._ Location: 2133 W. Lexington, Chicago, IL 60612 Days: Monday u2013 Friday Hours: 8:30AM u2013 4:30PM THIS POSITION IS IN THE CAREER SERVICE Qualifications: Two years of experience working in a community-based program or graduation from an accredited college or university with an Associateu2019s degree or higher.
