Summary The Nurse CSP Coordinator Registered Nurse (RN) is responsible for providing competent, evidence-based care to assigned patients. Provides clinical evidence-based services, interventions, education, process applications, and consultation for the caregiver and Veteran; community outreach; resource development; continuous quality improvement activities; and the evaluation of caregiver issues. Responsibilities Major Duties (To include but not limited to): Functions in support of caregivers, Veterans, and family members throughout the Veterans Affairs Medical Center (VAMC) and in community settings. Conducts interim and annual home visits to caregivers and Veterans approved in the program and provides assessments via phone and clinical video telehealth. Provides referrals and/or consults for Veterans Affairs (VA) services, as well as community-based social services and other non-VA entitlement programs. Serves as an advocate for patients, families, and caregivers, improving outcomes for the immediate practice setting. Promotes collaborative behavior and team efforts. Is confident of abilities, effectively initiates solutions and, is alert to new opportunities, techniques, and approaches. Responsible for developing and establishing linkages and collaborates on VA caregiver support efforts with community healthcare and social service agencies. Possesses advanced knowledge and understanding of the services and benefits available for caregivers from both federal, state, and private sector resources. Collaborates with the interprofessional team to manage resources, implement services and benefits to improve Veteran outcomes. Evaluates practice in an ongoing process, based on best evidence. Contributes professional nursing perspective in discussions with the interdisciplinary team.
