Employment Type:Full timeShift:Description:Trinity Health PACE Alexandria is seeking a compassionate and skilled Certified Occupational Therapy Assistant (COTA) to support our mission of keeping older adults living safely and independently in their homes and community.The COTA provides participant‑centered occupational therapy services under the supervision of a licensed Occupational Therapist, working within the PACE interdisciplinary team to address functional needs of frail elders through clinic‑based, home‑based, and community services.Position PurposeUnder the supervision of a licensed Occupational Therapist, the Certified Occupational Therapy Assistant (COTA) provides occupational therapy services to PACE participants to support independence, safety, and quality of life. Services are provided in the PACE center, clinic, participant homes, and community settings in coordination with the Interdisciplinary Team (IDT).Essential FunctionsImplements occupational therapy treatment plans as established by the supervising Occupational TherapistProvides therapy services to frail elderly participants in clinical, home, and community environmentsAssists participants with activities of daily living (ADLs), instrumental activities of daily living (IADLs), functional mobility, and transfersTrains participants and caregivers on adaptive equipment, safety strategies, and energy conservationObserves, reports, and documents participant progress and functional changesParticipates in interdisciplinary team meetings and care planningCompletes documentation in a timely manner in accordance with CMS PACE regulations, Trinity Health policies, and program standardsSupports fall prevention, dementia‑informed care, and participant‑centered servicesMinimum QualificationsGraduate of an accredited Occupational Therapy Assistant programCurrent Louisiana OTA/COTA license in good standing (or eligible)NBCOT certificationValid Louisiana driver’s license and reliable transportationAbility to work effectively with frail elderly participantsStrong communication, documentation, and teamwork skillsPreferred QualificationsExperience in geriatrics, PACE, home health, or community‑based careExperience working in interdisciplinary or value‑based care modelsFamiliarity with EMR/EHR systemsPhysical RequirementsAbility to assist with participant transfers and mobilityFrequent standing, walking, bending, and liftingAbility to travel locally between PACE sites and participant homesBenefitsMedical, dental, and vision insuranceRetirement plan with employer contributionPaid time off and paid holidaysContinuing education and professional development opportunitiesTrinity Health Mission StatementWe serve together in the spirit of the Gospel as a compassionate and transforming healing presence within our communities.Equal Opportunity StatementTrinity Health is an Equal Opportunity Employer.
